The ONE System: HSH Privacy Practice + ROI

The Department of Homelessness and Supportive Housing (HSH) is pleased to announce that as of July 1^st, 2019, we have implemented new Release of Information forms for the Homelessness Response System. Individuals who interact with the Homelessness Response System will also receive a new Notice of Privacy Practice, which informs them of how their information may be used or shared by HSH and its partner agencies. Please note that individuals at organizations funded both by HSH and by the Department of Public Health (DPH) should continue to use the DPH Notice of Privacy Practices (in Chinese; Spanish; Tagalog) and have the HRS Notice of Privacy Practices available upon request until otherwise directed.

HSH Notice of Privacy Practice

The Department of Homelessness and Supportive Housing Notice of Privacy Practice (in Chinese; Spanish; Tagalog) describes how personal information about clients may be used and shared by the City and County of San Francisco’s Department of Homelessness and Supportive Housing (HSH) and its provider organizations, and the rights clients have about their information. The privacy notice does not need to be signed, but does need to be provided to the client. See the opening section for situations where agencies are funded both by HSH and DPH.

Homeless Response System Release of Information

The Homeless Response System Release of Information (in Chinese; Spanish; Tagalog) explains how Client information is collected in the ONE System and used by or shared to housing and service providers (each, a “Partner Agency”, and collectively, the “Partner Agencies”), which includes community-based organizations and government agencies (such as the Department of Public Health or the Human Services Agency). This form must be signed by the client and uploaded into the ONE system. If a client verbally consents but refuses to sign the form, there is a place for that to be noted on the form. If a client refuses to provide verbal or written consent, a ONE system profile may not be created and the client will be limited as to the services they are able to receive from the Homeless Response System.

Human Services Agency Release of Information

The Human Services Agency Release of Information (HSA ROI) (in Chinese; Spanish; Tagalog) is a document signed by a client to give Department of Homelessness and Supportive Housing (HSH) and its partner agencies permission to access information about certain public benefits the individual may receive. For example, HSH needs to know if a client receives CAAP benefits as part of screening for eligibility for a CAAP funded (Care Not Cash) unit in CAAP.

Where can I upload ROI forms?

Signed release of information forms can be uploaded into ONE by clicking on the files tab under client profile.

To add a new file, select Add File Link on upper right corner of files section.

Select Release of Information Category and appropriate file name. Upload file and save.
